To contact our Medical Records Department, 
call 952-843-4100 or fax 612-454-2205. Email us at recordrequest@eosisrecovery.com for all EOSIS and Beauterre records. For Gateway Recovery Center in Brooklyn Park, email bprecords@gatewaydetoxmn.com and for the Inver Grove Heights location, email ighrecords@gatewaydetoxmn.com. EOSIS will respond to all records requests within 30 days of receiving them.
